Dear GGUSD Anthem Medical Plan Member:

Anthem recently advised that effective March 25, 2018, Prime Healthcare Services (PHCS) hospitals in Los Angeles, Orange, and San Diego counties will no longer be a part of Anthem’s provider network. This change is not within GGUSD’s control.

PHCS includes the following hospitals:

  • Centinela Hospital Medical Center
  • Garden Grove Hospital & Medical Center
  • Huntington Beach Hospital
  • La Palma Intercommunity Hospital
  • Paradise Valley Hospital
  • Sherman Oaks Hospital
  • West Anaheim Medical Center

Please note that this does NOT include individual physicians or physician groups affiliated with PHCS; many doctors have admitting privileges at more than one hospital and there are various other in-network facilities to choose from.

Please be assured that your health care coverage will not be interrupted as a result of this contract termination. Members currently receiving ongoing treatment or who are scheduled for care from one of the above affected hospitals should contact Anthem at 800-888-8288 (if enrolled in HMO) OR EBA&M at 855-322-7606 (if enrolled in PPO/EPO) regarding Continuity of Care to ensure that your care is not disrupted and allow for completion of your current course of treatment with your PHCS provider.

Dear GGUSD Employee,

Under the Affordable Care Act, employers and insurance carriers have certain guidelines and requirements they must follow. This year all full time eligible employees may receive two new IRS forms which can be used to help you complete your 2017 Tax Return. These forms provide proof of health insurance enrollment status for you in 2017. GGUSD will send Form 1095-C to you via US mail. Form 1095-B will also be sent from the insurance carrier to all employees who are enrolled in the Anthem Blue Cross HMO plan.
